Section C — Nature & Scope of Support

The undersigned individual ("Participant") voluntarily requests pastoral, spiritual, and/or educational care from the named minister or educator ("Provider"), operating under the auspices of Cage Free Voices ("CFV"). The support provided is:

(a)Faith-based, pastoral, and/or educational in nature — not clinical, psychological, or medical in scope;
(b)Offered voluntarily and may be discontinued by either party at any time without penalty;
(c)Governed by the CFV Neurotheological Discernment Framework and its associated ethical standards;
(d)Not a substitute for licensed mental health therapy, psychiatric evaluation, or medical treatment.

Section D — Informed Acknowledgments (All Required)

Voluntary Participation

I understand that my participation in any and all ministry support sessions is entirely voluntary. I may withdraw my consent and discontinue participation at any time, without coercion, pressure, or consequence to my standing within any faith community.

Scope of Care & Referral Acknowledgment

I acknowledge that the Provider is not a licensed medical professional, psychologist, or certified clinical counselor, and that the support offered does not replace professional clinical care. I consent to receive referrals to qualified licensed professionals when clinical symptoms are identified.

Spiritual Ministry Disclosure

I understand that any spiritual intervention will only be conducted with my explicit, ongoing, and revocable consent. The Provider is obligated under CFV protocols to operate under the leadership of the Holy Spirit, and spiritual engagement will not proceed if indicators of spiritual unpreparedness are present.

Confidentiality & Mandatory Reporting

I understand that information shared is held in confidence to the extent permitted by law. Mandatory reporting obligations exist and will be honored in cases involving imminent harm to self or others, child abuse, elder abuse, or other legally required conditions.

No Guarantee of Outcome

I acknowledge that Cage Free Voices and its practitioners make no guarantee regarding specific outcomes, healing, or spiritual results. Support is offered in good faith according to the CFV framework, and no specific result is promised or implied.

Release of Liability

To the fullest extent permitted by law, I release and hold harmless Cage Free Voices, its leadership, ministers, educators, and affiliated organizations from any and all claims arising from my participation in ministry support sessions rendered in good faith under CFV protocol.

Document Retention Notice

A signed copy must be retained by the Provider for a minimum of seven (7) years from the date of last session, or as required by applicable law.
